FFHS Athletic Forms and Policies

Athletic Registration

In order to be eligible to participate in a Fall, Winter, or Spring sport at FFHS, you must meet the following criteria:

-Have turned in a current physical form to Ms. Baldauf

-Have completed our online Athletic Packet at www.familyid.com (one time per school year)

-Provide proof of insurance – either through personal/family plan or school purchased plan

-Have passed 5 out of 7 units for previous semester (year-long classes count as 1 unit; semester classes count as 2 units)

 

  1. Visit www.familyid.com and click on create account in the top right corner

  2. Input the account owner’s information; check the box “agree to the FamilyID Terms of Service”, and click on the green create account button

  3. You will get a verification e-mail sent to the e-mail you just registered with; open the e-mail and click the link attached. You will now be able to log in to your account.

  4. Once you log in to your FamilyID Dashboard, type in “First Flight High School Athletics”. Click on the Program named “2017-2018 First Flight High School Athletic Registration”

  5. The Parent/Guardian and the Student-Athlete must both be present at the time of completing the application in order to sign in their designated boxes  

  6. Click on the sport that your child intends to tryout for (this can be more than one if in multiple seasons)

  7. Complete the registration form entirely- YOU MUST PUT IN THE STUDENT’S INSURANCE POLICY NUMBER!

  8. At the end of the form hit the green Save and Continue button- make sure you hit the Submit Registration button on the next screen or it will not save

 

Anytime after you have submitted a registration but need to make a change to the information on the form (insurance policy number/carrier, medications, or change in contact info), go to the Registrations tab on the top blue bar. Click summary and then Edit. Be sure to save your changes by clicking the green Save and Continue button at the bottom of the form.  

 

Returning Users

  1. Visit www.familyid.com and click on Log In in the top right corner

  2. Once you log in to your FamilyID Dashboard, type in “First Flight High School Athletics”. Click on the Program named “2017-2018 First Flight High School Athletic Registration”

  3. The Parent/Guardian and the Student-Athlete must both be present at the time of completing the application in order to sign in their designated boxes  

  4. Click on the sport that your child intends to tryout for (this can be more than one if in multiple seasons)

  5. After selecting your child’s sport(s), scroll down to Participant Information. Find the drop down box that says Click here to SELECT or CREATE a participant. Select your child’s name with the most recent date and time stamp. This will populate the entire registration with their information from last year

  6. Update any information that may have changed over the course of the year such as their grade, contact information, medical history, or insurance policy number/carrier

  7. Both the Parent/Guardian and the Student-Athlete must sign in the designated boxes

  8. At the end of the form hit the green Save and Continue button- make sure you hit the Submit Registration button on the next screen or it will not save
